DECLARATION BY CLAIMANT

I acknowledge that Old Mutual may approve or decline this claim subject to the terms stipulated in the policy contract(s) and based on the information that has been submitted to Old Mutual in terms of the claim.

In the event that the details of the deceased or claimant on Old Mutual’s records differ from the updated details on this form, I confirm that these people are one and the same.

I further understand that any request for adding an insured person requires further evaluation by the processing centre and that Old Mutual will notify me of the outcome of the request.

I confirm that all the information on this form is true and accurate to the best of my knowledge.

I give consent to Old Mutual to confirm this information with any appropriate source.

I hereby absolve Old Mutual from any risk or liability if Old Mutual pays out to the wrong person or into a wrong bank account due to incorrect or incomplete information that I provided.

If you opened a Money Account: I confirm that the advantages (for example I may receive the money quicker) and disadvantages (for example, that Money Account is a transactional account like any other and charges apply) were explained to me. I choose to have the money from a successful claim paid into this account. I know the claim can only be processed when Old Mutual has received all the claim requirements.
