• I understand that a security cession form must be completed for security cessions and a transfer of ownership form must be completed for a policy owner change in addition to this request.

• I also understand that if a security cession is being cancelled, then a Cession cancellation letter from cessionary which includes the policy number, client's name and security code is required.
