IMPORTANT NOTES

• I am/we are the legal holder(s) of the above mentioned policy.

• My/Our estate has not been sequestrated and is at present solvent to the best of my/our knowledge and belief.

• I/We have not ceded or pledged the said policy either by Antenuptial Contract or otherwise, save as regards any existing cession in favour of the Old Mutual as security for a loan.

• I/We hereby absolve Old Mutual from any risk or liability resulting from the proceeds being deposited into an incorrect bank account in the event that I supplied the incorrect banking details. Applicable at maturity only: The Nomination of Beneficiary(ies) (if any) is hereby cancelled.
